Registration of Births

Registration of a Birth outside of Sri Lanka and obtaining the Citizenship Certificate

1. Duly filled Application (if the birth registration within the three months from the Birth [Particulars for Registration of a Birth – CR01] and if it is after three months [24^th Declaration and Particulars for Registration of a Birth – CR01] should be filled. Please follow the guideline to complete the CR01
2. Duly filled application for Citizenship
3. Birth Certificate issued by the Indonesian authorities along with its English translation
4. Parents’ original Birth Certificates (English translations are not accepted)
5. Parents’ original Marriage Certificate (English translation is not accepted)
6. Parents’ original Passports
7. Declaration by father (If father is a foreigner, mother should give it)
8. Original Stay Permits of Parents
