Originals of the below-listed documents, together with 2 sets of photocopies should be submitted with the application (originals will be returned after due verification)
ELIGIBILITY
Sri Lankan Nationals living in Indonesia
Requirements
- Duly filled passport application form K (Forms are available to be downloaded through www.immigration.gov.lk)
- Current Passport with valid Residence Permit
- Four passport-size (4.5cm x 3.5cm) color photographs should be clear, good quality, fresh (current picture) without spectacles and with open ears, with white background/light color.
- Original birth certificate (translations are not accepted)
- If the applicant was born outside of Sri Lanka, the Citizenship Certificate
- If the applicant is a citizen by registration, the Citizenship Certificate
- National ID card along with a photocopy
- Maximum of 6 months before the passport expires
- Application fee – IDR 2,230,000
Note: Applicant should submit the application along with previous N/M series passport by him/herself

LOST/MISSING PASSPORTS
The information below is for the charges for the issuance of Sri Lankan Travel Documents under the circumstance in which the original travel documents went missing:
- Lost with a copy – US$ 373/IDR 5,260,000
- Lost without a copy – US$ 473/IDR 6,670,000
- Non-machine readable Passport (NMRP) – US$ 54/IDR 765,000
- Endorsement/Amendments (per amendment) – US$ 169/IDR 2,370,000
